With Medicare and Medicaid, PACE provides no-cost:
Medical care, including dental, vision
and hearing
Help managing medications
A nurse available to call anytime,
day or night
Rides to all visits and appointments
A social center with activities and
therapy gym
Meals and support at home
*If you only have Medicare coverage, you may pay a fee. Participants may also pay privately for PACE services.

Who qualifies for PACE?
At least 55 years old
Certified by the Commonwealth of Kentucky to need nursing home-level care
Able to live safely in the community at the time of enrollment with PACE services
Residents of Bullitt, Grayson, Hardin, Hart, LaRue or Nelson Counties

Participants must receive all health care, including primary and specialist services (other than emergency services), from Care Guide Partners PACE (GCPP) or other authorized contracted providers. Participants may be fully and personally liable for the costs of unauthorized, out-of-network care or services. Participation is voluntary and participants may disenroll from PACE without cause at any time. For those dually eligible, a disenrollment date will be coordinated between Medicare and Medicaid.
