Mayor’s proclamation underscores Care Guide Partners’ commitment to comprehensive senior care through the Program of All-Inclusive Care for the Elderly (PACE).

(ELIZABETHTOWN, KY) — In celebration of National PACE Month, Elizabethtown Mayor Jeff Gregory issued a proclamation on September 30, recognizing Care Guide Partners PACE, an affiliate of Hosparus Health, for its dedication to providing comprehensive Program of All-Inclusive Care for the Elderly (PACE) services to seniors in Bullitt, Grayson, Hardin, Hart, LaRue, and Nelson counties.

“National PACE Month is an opportunity to reflect on the remarkable impact PACE has on our community,” said Judith Parsons, Executive Director of Care Guide Partners PACE. “By integrating health and social services, we empower seniors to live independently in their homes while receiving the care they need to do so safely. Mayor Gregory’s proclamation helps raise awareness of the vital support we provide to seniors and their family caregivers in and around Elizabethtown.”
 
What is PACE?
PACE (Program of All-Inclusive Care for the Elderly) is a Medicare and Medicaid-funded program designed to help adults aged 55 and older age safely in place. At the William B. and Janet B. Schmidt PACE Center in Elizabethtown, participants receive a comprehensive range of medical, social, and support services, coordinated by Care Guide Partners’ interdisciplinary team of senior care specialists.
“Care Guide Partners PACE tailors services to meet the unique needs of each senior, offering preventive and specialty medical care, recreational and rehabilitation therapies, nutrition planning, dentistry, and social support,” said David W. Cook, President and CEO of Care Guide Partners. “Many of these services are conveniently provided under one roof at the PACE Center, with roundtrip transportation included — making life easier for participants and their families.”
Cook added, “By helping participants maintain their independence, PACE exemplifies our nonprofit organization’s mission to enhance quality of life in our community. We are honored by Mayor Gregory’s recognition this PACE Month and are excited to showcase how this program’s seamless, personalized care coordination supports seniors and their families in the region.”
