Who’s eligible for PACE?

PACE is for independent seniors who may need assistance in performing activities of daily living but wish to still live in their homes. To qualify for Care Guide Partners PACE services, you must be:

  • At least 55 years old
  • Certified by the Commonwealth to need nursing home-level care
  • Able to live safely in the community with help from PACE services
  • Live in Bullitt, Grayson, Hardin, Hart, LaRue, or Nelson counties in Kentucky

The Centers for Medicare and Medicaid Services mandates eligibility criteria for all PACE providers.

What does PACE cost?

The cost of the PACE program depends on your Medicare and Medicaid eligibility. Services are 100% covered for participants with both Medicare and Medicaid. Seniors with Medicare may have to pay a monthly premium but do not have to pay deductibles or copayments. Private pay options are available for PACE participants who do not have Medicare or Medicaid.

Because all your health care services must be received or coordinated by your PACE team, participants may be fully and personally liable for the cost of services not authorized by Care Guide Partners PACE.
