PACE participants receive 24/7 comprehensive health and supportive services across a range of settings: at our local PACE Center, in the home, and in the community. Transportation is always included to ensure access to these services.

Complete Care Services

PACE provides care to seniors who wish to continue living in their own homes despite chronic health needs. Each PACE participant has a team of medical experts dedicated to providing personalized healthcare, coordinated at our PACE Center, conveniently located near your home.

Participants receive all health care, including primary and specialist services (other than emergency services), from Care Guide Partners PACE (GCPP) or other authorized contracted providers. Participants may be fully and personally liable for the costs of unauthorized, out-of-network care or services.

Medical Services

Going from one medical appointment to another can be exhausting for everyone. To make life easier, our PACE Center provides many services under one roof.

All medical care is provided by our expert team of primary care providers (PCPs), nurse practitioners and registered nurses. All are specialists in geriatric medicine. Your PACE onsite team performs ongoing assessments and treatments to support your unique medical needs.

Specialty Care

Specialty medical care including dental, podiatry, optometry, audiology services and medical supplies are available through the PACE network of medical specialists. The PACE Medical Director and PACE interdisciplinary team must approve referrals for specialty care as medically necessary.

Nutrition Planning

Nutrition is a fundamental building block of continued health and independence. That’s why PACE employs registered dieticians who work closely with PACE participants to assess their nutritional needs and make recommendations that are built into personalized care plans for everyone in the program. A PACE dietary assessment reviews nutritional status, needs, risks for disease, and any current nutritional challenges, including in-home nutrition access problems and food insecurities. From there, PACE dieticians offer individual counseling that empowers participants to make informed choices about eating healthy at home and optimizing their nutritional health.

In addition, we offer a light breakfast and hot lunch meal at the PACE Center every day. Home-delivered meals can be arranged as needed.

Recreation Therapy

Keeping your mind and body busy are also keys to staying independent as possible. Our recreational therapists and the PACE Center Adult Day Program provide a fun and engaging environment with a variety of activities and hobbies designed to help people socialize and improve their physical and cognitive capabilities. Some sample Adult Day Program activities might include:

  • Arts and crafts
  • Bingo and trivia
  • Computer access and lessons
  • Dance parties
  • Day trips
  • Chair yoga
  • Holiday and cultural celebrations
Rehabilitation Therapy

Care Guide Partners PACE provides different types of rehabilitation therapy for individual needs. Focused on healing and restoration, we offer occupational, physical and speech therapies with experienced therapists who work to rebuild your strength, mobility and overall ability. We also provide durable medical equipment including walkers, wheelchairs, grab bars, and shower chairs.

Transportation

PACE transportation services are included in the program to relieve the stress of getting to and from coordinated appointments for you and your family. Our experienced drivers are trained to meet the specialty needs of seniors and provide you with all necessary accommodations to ensure your safety and comfort.

In-Home Assistance​

When day-to-day activities become difficult, living at home doesn’t have to mean being alone. The PACE in-home support team makes home visits to help with daily tasks such as bathing and dressing, meal preparation, laundry, grocery shopping, or light housekeeping.

Social Services

Our skilled social workers advocate for your needs and provide guidance and support for you and your family members. Social services such as counseling, access to legal or financial resources, and caregiver education are available to help you make the best possible decisions. Serving as the bridge of communication between PACE participants and their care team, our social workers are here to help make your wellness journey easier.

Spiritual Care

Care Guide Partners PACE also offers peace and comfort with our spiritual care services. Our chaplains and spiritual companions provide support and help in finding meaning, hope, and spiritual connection if you desire. Spiritual care and healing in mind, body and spirit can contribute to overall wellness, ultimately improving the quality of life for PACE participants and their families.
