    • Who are PACE services for?

    PACE serves adults who are:

    • At least 55 years old
    • Certified by the Commonwealth to need nursing home-level care
    • Able to live safely in the community with help from PACE services
    • Living in Bullitt, Grayson, Hardin, Hart, LaRue, or Nelson counties in Kentucky
    • What services are included with PACE?

    • General

    PACE provides the entire continuum of medical care, and long-term services and support necessary for our participants to achieve the highest quality of life possible while living in their homes and communities. PACE services include:

    • Primary and specialty medical care
    • In-home health and personal care services
    • All necessary prescription drugs
    • Specialty medical care: audiology, dentistry, cardiology, optometry, podiatry and speech therapy
    • Respite care and family caregiver support
    • Transportation to all services and appointments
    • Adult day services, including nursing, meals, nutritional counseling, social work, personal care, and physical, occupational and recreational therapies
    • Hospital and nursing home care, when necessary

    All Medicaid and Medicare services are covered through PACE, plus more. Participants receive all health care including primary and specialist services (other than emergency services) from Care Guide Partners PACE or an authorized contracted provider. Participants may be fully and personally liable for the costs of unauthorized, out-of-network care or services.​

    • What is there to do at the PACE Center?

    Our goal is to offer comprehensive care by a coordinated team committed to improving our participants’ quality of life by empowering you to take an active role in your wellness.

    Our Elizabethtown PACE Center offers a primary medical care clinic, dental care, meals, therapy, social support and counseling, and an Adult Day Center that provides opportunities for socialization and recreation in one convenient location. Transportation to and from the PACE Center is available to everyone in the PACE program. 

  • PACE® (Program of All-Inclusive Care for the Elderly) is a Medicare and Medicaid program designed to help people meet their healthcare needs in the community instead of going to a nursing home or other care facility. PACE provides personal, coordinated medical services and social support to help aging adults stay independent and safely living in their homes for as long as possible.

    Care Guide Partners PACE provides all the care and services covered by Medicare and Medicaid, as well as additional medically necessary care authorized by the interdisciplinary team. This dedicated team of healthcare professionals who specialize in aging and chronic conditions creates a customized care plan to ensure holistic and optimal health, safety, and well-being for program participants. PACE includes coverage for prescription drugs, doctor care, transportation, home care, check-ups, hospital visits, and even nursing home stays whenever necessary. With PACE, a participant’s ability to pay will never keep them from getting the care they need to continue living in the community safely.

    • What is “Nursing Home-Level of Care”?

    “Nursing Home-Level of Care,” or “Nursing Facility Level of Care,” is a measured level of care needs that must be met for Medicaid nursing home admissions and varies from state to state.

    For Kentucky residents, this level of care is defined in 907 KAR 1:022. Status decisions are based on medical diagnosis, care needs, services and health personnel required to meet these needs, and the feasibility of meeting these needs through alternative institutional or noninstitutional services.

    • What happens to my Medicare/Medicaid coverage if I join PACE?

    When you enroll in PACE, you keep the benefits offered by Medicare and/or Medicaid and become eligible for additional benefits offered by Care Guide Partners PACE.

    • What happens to my health insurance if I join PACE?

    • Insurance

    Once you join PACE, Care Guide Partners becomes your health insurance provider. Care Guide Partners also coordinates and pays for all of your necessary medical care. 

    Please note that Care Guide Partners PACE participants must receive all needed health care, including primary care and specialist physician services (other than emergency services), from Care Guide Partners or a person or company authorized by Care Guide Partners. Care Guide Partners PACE participants may be fully and personally liable for the costs of any out-of-network services not authorized by the Care Guide Partners PACE program.

    • What happens if a person wants to leave PACE?

    • General

    Any PACE participant is free to disenroll from the program without cause at any time. Participants must contact the PACE organization directly to voluntarily change their status and cannot enroll or disenroll at the Social Security Administration office. The disenrollment date will be coordinated between Medicare and Medicaid for a dually eligible participant.

    No disenrollment will become effective until the participant is appropriately reinstated into other Medicare and Medicaid programs and alternative services are arranged. The PACE organization must continue to provide all needed services, and the PACE participant must continue to use the PACE organization’s services and pay any premiums, until the date the enrollment is actually terminated. Program enrollment and disenrollment usually occur at the beginning of each month.

    • Is transportation provided for participants?

    • General

    Yes, PACE provides all medically-necessary transportation for participants! Our medical transportation specialists pick up participants from their homes and safely transport them to and from all PACE Center activities, medical appointments, and other community support services coordinated by the PACE team.

  • PACE is a Medicare and Medicaid program, and what you pay depends on your financial situation.

    • If you qualify for Medicare, all PACE services are paid for by Medicare.
    • If you are also eligible for Kentucky’s Medicaid program, PACE services are covered at no additional cost.
    • If you do not qualify for Medicaid, you will be charged a monthly premium to cover the long-term care portion of the PACE benefit and a premium for Medicare Part D drugs.
    • If you do not have Medicare or Medicaid, you can pay for PACE privately.
    • There are no deductibles, copayments, or coinsurance for any care and services approved by our team of healthcare professionals.

    • How do I schedule an evaluation?

    • Do I need to change my physician if I join PACE?

    PACE participants receive a primary care provider (PCP) and have access to a full medical team at our local PACE Center. This PCP works with other healthcare professionals to coordinate your care and ensure your medical needs are met, making life easier for you and your family caregivers. Our medical teams are experts in senior health, dedicated to providing personalized care tailored to each participant we support.
