As a Program of All-Inclusive Care for the Elderly (PACE) provider, we work to empower people aged 55 and older with the support they need to stay in the homes they know and love as they age.
We proudly serve residents of Bullitt, Grayson, Hardin, Hart, LaRue, and Nelson counties.
PACE is a nationally recognized full-service healthcare program designed to help you age safely in place. We provide all medical care, senior services, transportation, and in-home support necessary to maintain your lifestyle. If you or a loved one needs some support to continue living independently, we’re here to help.
Our team of aging specialists will work with you and your family to create a customized plan tailored to your needs and goals. The PACE team coordinates the delivery of all services outlined in your plan. We schedule all medical appointments, provide transportation, social services, meals, and any other day-to-day assistance required based on your unique level of needs.
PACE services are 100% covered for Medicaid participants and those dually eligible for both Medicare and Medicaid. Those with Medicare who do not qualify for Medicaid may be charged a monthly premium for PACE. Private pay options are also available.
