Center brings the comprehensive services of the national Program of All-Inclusive Care for the Elderly (PACE) to the region, supporting adults who wish to maintain their independence at home as they age.

(ELIZABETHTOWN, KY) — On March 27, Care Guide Partners PACE, an affiliate of Hosparus Health, proudly unveiled its new $11 million, 18,000-square-foot William B. and Janet B. Schmidt PACE Center at 109 Buffalo Creek Drive in Elizabethtown. The grand opening event welcomed visitors to explore this state-of-the-art facility and learn more about the newest program option available for area seniors. The PACE Center offers a comprehensive range of health and wellness services designed to help seniors stay living independently in their own homes for as long as possible.

The grand opening event welcomed visitors to explore the facility and learn more about the national Program of All-Inclusive Care for the Elderly (PACE) – the newest healthcare option for area seniors. Through the PACE Center, Care Guide Partners offers a comprehensive range of health and wellness services designed to help older adults continue living in their own homes for as long as possible. This all-inclusive program is now accessible to qualifying residents of Bullitt, Grayson, Hardin, Hart, LaRue, and Nelson counties.

During the ceremony, Care Guide Partners and Hosparus Health’s President and CEO David W. Cook, expressed his excitement: “As we inaugurate the William B. and Janet B. PACE Center, we embark on a new chapter to revolutionize healthcare accessibility and enhance the quality of life in the communities we serve. Here, our PACE team delivers all the medical, social, nutritional, and support services individuals need to remain safely in the homes they know and love as they age, for as long as possible.” Cook further stated, “The personalized care provided by PACE could be the solution many older adults and their families are looking for. This initiative reflects our 45-year commitment to serving with compassion, integrity, and innovation.”

What is PACE?
PACE is a national Medicare and Medicaid-funded healthcare model designed to help adults 55 and older maintain their independence and age in place safely while residing in the community. Developed in 1971, PACE is a complete system of managed healthcare and service delivery that supports seniors and their caregivers by addressing the challenges often encountered while accessing medical care from multiple providers in different settings and across health systems.

Personalized, Holistic Care for Seniors
The PACE model provides all the medical care and support services necessary to meet each participant’s care needs. Eligible individuals who enroll in the program agree to receive all their non-emergency services, including primary and specialist care, through the PACE organization. A PACE physician becomes their primary care provider and works with a team of specialists to develop a personalized plan of care to ensure participants have the support they need to age in place safely: 24 hours a day, seven days a week, 365 days a year. Each plan is tailored to their unique medical, social, physical, and functional needs. All care is coordinated and monitored by the team at the PACE Center in collaboration with individuals and their family caregivers.

PACE Services
PACE functions as both the healthcare provider and the healthcare plan for participants and truly is a one-stop shop for all Medicare and Medicaid-covered services. This means that the PACE team works directly with individuals to make decisions for their necessary care. Services are provided at the PACE Center, in the home, and even in the community. Care Guide Partners PACE is responsible for the quality and cost of all care provided by its established network of providers, including specialist and hospital services. Participants who seek care outside of the PACE network may be personally liable for the cost of that care.* The program provides roundtrip transportation from participants’ residences to all appointments managed by PACE, whether at the Center or with an authorized PACE provider, to make accessing care as easy as possible.

The PACE Center
Many of Care Guide Partners PACE services are conveniently offered at the new William B. and Janet B. Schmidt PACE Center in Elizabethtown, including primary medical care at the on-site clinic, adult day programming, occupational, physical, and recreational therapy, behavioral health, specialty services, hot meals, nutritional counseling, social activities, and more.

Cost of Program for Participants
Individuals enrolled in PACE will have no copays or deductibles for the program’s services. PACE covers the cost of all care and services normally covered by Medicare and Medicaid, as well as medically necessary care authorized by the participant’s care team that may not normally be covered by Medicare and Medicaid. Participants who are only covered by Medicare (and not Medicaid) will have a monthly premium that will be discussed upon enrollment in the program. Private pay options are also available.

To qualify for Care Guide Partners PACE, participants must be:

  • At least 55 years old
  • Certified by the Commonwealth of Kentucky to need nursing home-level care
  • Able to live safely in the community at the time of enrollment with PACE services
  • Residents of Bullitt, Grayson, Hardin, Hart, LaRue, or Nelson counties

*Because all health care services must be received at or coordinated by the PACE Center, participants may be fully and personally liable for the costs of services not authorized by PACE. A PACE participant may voluntarily disenroll from the program without cause at any time. The disenrollment date will be coordinated between Medicare and Medicaid for dually eligible participants.
