About Care Guide Partners PACE

Care Guide Partners PACE, part of the Hosparus Health family, is working with state and federal agencies to bring this innovative, nationally recognized model of care to the Elizabethtown region. Construction on the Care Guide Partners PACE Center, which serves as the hub for the program, is on track for completion by early 2024. 

The estimated start-up cost to bring Care Guide Partners PACE to Elizabethtown is $11 million. Of this total, we need to raise $3 million through fundraising efforts and donor support. Hosparus Health is working on the organizational level to raise funds to create an endowment, ensuring the long-term financial stability of this project. We look forward to sharing more with you soon about how this innovative program allows our nonprofit organization to build on our 45-year mission and enhance the quality of life for those we serve.
