The grant supports the nonprofit organization’s new Program of All-Inclusive Care for the Elderly (PACE) Center in Elizabethtown, with programming for area seniors.

(LOUISVILLE, KY) — Care Guide Partners, an affiliate of Hosparus Health, is proud to announce a transformative $500,000 lead grant from the Legacy Foundation of Kentuckiana. This generous contribution supports the launch of the Care Guide Partners PACE Center, situated at 109 Buffalo Creek Drive in Elizabethtown. Scheduled to open next quarter, the PACE Center will increase accessibility to comprehensive healthcare services for seniors in and around the region.

Hosparus Health, renowned for hospice, palliative care, and community grief counseling, is working with state and federal agencies to bring the national Program of All-Inclusive Care for the Elderly (PACE) to Hardin and surrounding counties through its affiliate brand, Care Guide Partners. PACE is a Medicare and Medicaid-funded program that enables individuals aged 55 or older to maintain their health and independence from the comfort of their homes.

An interdisciplinary team of specialists at the PACE Center collaborates with participants and their families to develop a personalized plan of care, addressing each individual’s complete medical, social, physical, and functional needs. The team monitors and coordinates all care, with many services conveniently offered at PACE Center, including primary medical care, adult day programming, therapy, behavioral health, specialty services, meals, nutritional counseling, social activities, and more. The program provides roundtrip transportation from participants’ residences to all appointments managed by PACE to ensure accessibility for seniors and their families.

A 2022 feasibility study conducted in Kentucky for the organization by the Palmetto PACE Advisory Group indicates approximately 2,086 households in the Elizabethtown region could benefit from PACE program services. The Care Guide Partners PACE initiative requires $11 million in startup costs, with $3 million to come from fundraising and donor support. The Legacy Foundation of Kentuckiana formally presented its $500,000 lead gift at the December 2023 Care Guide Partners board meeting.

“We are grateful for the generosity of the Legacy Foundation of Kentuckiana as our organization expands our services to bring this exciting program to Hardin and surrounding counties through the new Care Guide Partners PACE Center,” stated Tawanda Owsley, SVP/Chief Development and Marketing Officer at Care Guide Partners and Hosparus Health. “This state-of-the-art, 18,000 square-foot facility will serve as a hub for PACE services and allow our organization to support an average of 100-120 seniors each day at the PACE Center, with room to accommodate up to 250 as the program grows.”

We look forward to seeing the impact it will have for seniors and families in Elizabethtown and its surrounding counties.”
